Welcome to the enrollment section of our daycare! Below you'll find all the necessary forms and information to get your child registered and ensure a smooth start. Please download, review, and complete the required documents. Don’t hesitate to reach out if you have any questions.

• OCFS Childcare Rates: This document, provided directly by the Office of Children and Family Services (OCFS), offers detailed information on childcare rates for various types of providers. Please note, our rates are listed under Family Day Care Home and Group Family Day Care Home, and we only offer weekly rates, not daily or part-day options. 

• Routines/Daycare Schedule: A detailed daily schedule that shows the times for consistent routines such as meals, play, naps, and structured activities. While the times and structure remain consistent, the specific themes will change weekly to align with our current thematic unit.

• Daycare Contract 2024-2025: Please read carefully, sign, and initial each page. This contract outlines the terms and conditions for the upcoming year, including important policies, responsibilities, and fees. It details what we promise to provide and what we need from you as a parent. Be sure to review all details thoroughly before signing. 

• Daycare Enrollment Form: Complete this form to secure your child's spot in the daycare. The registration includes essential information such as your child’s details, emergency contacts, medical information, and agreements for participation in activities and neighborhood trips. Please ensure all sections are filled out accurately.

• Medical Statement: This form must be completed by a healthcare professional to ensure your child’s medical information is up to date. It includes details about immunizations, including exemptions, lead screenings, and TB tests. The form also covers any allergies, medications, special diets, and other medical or developmental conditions requiring attention. Please ensure all sections are filled out and any required documentation is attached.

• Special Needs Health Care Plan: For children with chronic physical, developmental, behavioral, or emotional conditions, this form documents the required care, any additional training for caregivers, and necessary accommodations.

• Medication Administration Form: If your child needs medication while in our care, please complete this form to provide consent, instructions, potential side effects, and administration details. Note that without Medication Administration Training (MAT) staff, we can only administer specific over-the-counter (OTC) medications with prior written consent. A separate form is needed for each medication. For prescription or specialized treatments, please arrange with a licensed health professional.
